ROI (Release of Information)

This page provides an overview of the Mono County System of Care Release of Information (ROI): Multi-Agency Consent and Authorization to Disclose, Exchange, and Use Information and Records. This ROI is used by many partner agencies in Mono County and is designed to streamline care coordination while ensuring that clients give fully informed consent. The current ROI will have a revision date of 7/1/26 at the bottom of each page.

In the documents section of this page, you will find an English and a Spanish copy of the ROI. Below, you will also find three training videos. It is strongly recommended that you watch these videos before utilizing the new ROI.

*Participating agencies are welcome to use these videos as examples of how to explain the ROI, or they can play the video for clients rather than explaining the ROI themselves.

 

Notes for agencies:

  • In the uncommon case that an individual wishes to revoke their ROI, please direct them to the original agency where paperwork was completed. It is the originating agency’s responsibility to keep a copy of this ROI for 12 months and to inform all parties initialed that the ROI needs to be revoked.
  • If a copy of this ROI is presented to you, it is your responsibility to understand your agency’s policy on releasing information. Not every agency listed on this ROI is able to release information using this ROI and may require the client to complete their own ROI before releasing information.
  • Once signed, it is recommended that you provide a copy of this ROI to any partners you are actively coordinating with to help reduce administrative burden for the individual who signed the ROI.
  • This ROI replaces the Children's System of Care ROI. Agencies who present that ROI to MCBH in the future will be asked to complete this ROI instead.
  • This ROI may be updated from time to time. It is recommended that you visit this page to download the latest copy.
