Mono County Health and Human Services works with community members, healthcare providers, schools, local organizations, and regional partners to understand the health needs of people in the county.

Together, partners review data and community feedback to understand what is working well, what needs improvement, and what actions can help people live healthier lives.

How does this process work?

This work happens through two key planning processes:

Community Health Assessment (CHA)

A review of local data, community feedback, and health information that helps identify the biggest health needs and gaps in care in Mono County.

Community Health Improvement Plan (CHIP)

A community action plan created with local partners that outlines goals, strategies, and actions to address the health priorities identified in the CHA.

Together, the CHA and CHIP guide Mono County’s public health planning, partnerships, and investments in programs and services.


2026 Community Health Improvement Plan (Now Available!)

Using community feedback and local data, three priority areas were identified to help improve health in Mono County:

Access to Care

Improve equitable access to comprehensive and culturally responsive healthcare services across Mono County.

Key strategies include:

  • Expanding mobile dental services and school-based oral health programs
  • Launching community-based preventive care clinics
  • Exploring options to establish local vision care services

Mental and Behavioral Health

Support mental and emotional well-being by improving access to behavioral health resources and strengthening prevention and early intervention.

Key strategies include:

  • Implementing routine depression screening (PHQ-2) across Public Health services
  • Expanding food access initiatives for low-income and Medi-Cal residents
  • Supporting home visiting services that promote family well-being and early childhood development

Maternal, Child, and Infant Health

Strengthen the maternal and child health system by improving access to prenatal care, preventive services, and early childhood support.

Key strategies include:

  • Increasing well-child visits and childhood immunizations
  • Strengthening perinatal care coordination and referral pathways
  • Expanding outreach and education for pregnant and postpartum individuals

Implementation and Community Partnerships

Carrying out the CHIP will focus on collaboration between public health, healthcare providers, community organizations, schools, and Medi-Cal managed care plans.

Priority implementation approaches include:

  • Mobile and community-based service delivery
  • Expanding services in northern and southern Mono County communities
  • Cross-sector coordination for maternal, child, and behavioral health
  • Addressing social drivers of health such as food insecurity

Community partners and residents are encouraged to participate in CHIP initiatives and help advance shared health goals.
